Payment for services is due at the time services are rendered. We accept cash, checks, MasterCard, Visa, American Express, and Discover. We also provide financial arrangements for extensive treatment. Using CareCredit and Greensky Credit, you may qualify for low- or interest-free payments. Please call our office for more details. Returned checks and balances older than 30 days may be subject to additional collection fees and interest charges. Charges may also be made for broken appointments and appointments canceled without 24 hours notice.
Our office accepts Delta Dental PPO, Aetna PPO, Cigna PPO, Humana PPO, United Healthcare PPO, and Smile Select Savings Plan. However, we do not belong to any other insurance “plan lists” for HMO, DMO, PPO, and discount plans. Our doctor’s treatment recommendations are for your best long term results. Insurance plans often allow for the lowest cost treatment or have yearly limitations of payments.

To process your insurance claim, we will need your insurance card or form and your signed release. We will accept assignment of insurance benefits once it is verified that your insurance will pay us. If you have dual insurance, we will accept assignment on the primary, but expect you to pay the estimated balance and be reimbursed by your secondary insurance. We will estimate your coverage as closely as possible, but until we actually receive payment from the insurance company, it remains an estimate.
